A medication review (also known as pharmacist care plan) allows you to have one-on-one time with a pharmacist to review your medications and medical conditions to help you reach your personal health goals.
We ensure that your medical conditions are being optimally managed by your medications and include a review of your current labs, vaccinations, lifestyle, and much more.
No two pharmacist care plans are the same as they are tailored to your unique healthcare needs.
We also encourage you to bring us any measurements or tests done at home, such as blood glucose or blood pressure readings to aid in our overall assessment.
This service is offered once per year and is covered by Alberta Health at no cost to you if you have certain medical conditions such as asthma, COPD, hypertension, diabetes, and some others, and comes at no cost to you.
